■Procedure
1. Download and print out the following PDF documents
Change of registered information form
The sample of how to fill out the form (in Japanese)
Postage-free return envelope
2. Fill out the above form and enclose the required identification document if needed
3. Send them back to SBI Shinsei Bank by postal mail

■Enclosed Documents
・For changing the registered personal seal or signature (normal change)
No additional documents are required

・For changing the registered personal seal or signature (when lost or forgotten)
A copy of an official document that verifies your name, date of birth and current address
Ex) Both sides of your Residence Card, etc.

■Processing time
・The procedure will be completed within approximately one week after the documents arrive at SBI Shinsei bank.
・If there are any issues with the submitted documents, a notification letter will be sent to you.

■Instructions for filling out the form
1. Fill in the required fields at the top of the form
Please enter the date, branch number, account number, and registered name in the respective fields at the top of the form.


2. Stamp or provide your signature in the "Registered Seal or Signature" field
Please affix your registered seal or provide your signature in the "Registered Seal or Signature" field at the top of the form.
If you have lost or forgotten your registered seal or signature, please use a new seal or provide your signature.


3. Circle the Changes in the Middle Section of the Form
In the middle section of the form, please circle the items that you wish to change.
*If there are any changes to other items such as the address or name, please also circle the respective items.



4. Fill in Only the Items with Changes at the Bottom of the Form
At the bottom of the form, please fill in only the "New Seal or Signature" field.
*If there are any changes to other items such as the address or name, please also fill in the respective fields.
