・Your Account Number (as shown on your cash card)

・Your Date of Birth

Receive Acknowledgement email for your Request
Receive an email, titled as below to from the email address you sent to us at STEP 3 .
Subject:【SBI Shinsei Bank】Acknowledgment (email address) of your Procedure Request Form
At this stage, we will not able to confirm if the email you entered matches your registered alert email address. In case the email address does not match, you will not be able to proceed with this procedure form.
Regarding our process of WebForm requests:
◆ Please note that it may take several days for the completion of your request, or there may be cases in which we are not able to process your request through this request form.
◆ Depending on the details of the request and the status of the account, the bank may contact you by email or telephone.
◆ We will basically contact you between 9am to 5pm.
