1. Download and print out the following PDF documents
Change of registered information form

Postage-free return envelope
2. Fill out the above form and enclose the required identification document
3. Send them back to SBI Shinsei Bank by postal mail
■Enclosed Documents
・For address change
A copy of an official document that verifies your new address, name, and date of birth
Ex) Both sides of your Residence Card, etc.
・For name change
A copy of an official document that verifies your old and new names, address (new address if changing), and date of birth
Ex) Both sides of your Residence Card, etc.
・For phone number or Email address change only
No additional documents are required
■Processing time
・The procedure will be completed within approximately one week after the documents arrive at SBI Shinsei bank.
・For name changes, a new cash card will be sent after the procedure is completed.
・If there are any issues with the submitted documents, a notification letter will be sent to you.

■Instructions for filling out the form
1. Fill in the required fields at the top of the form
Please enter the date, branch number, account number, and registered name in the respective fields at the top of the form.
2. Stamp or provide your signature in the "Registered Seal or Signature" field
Please affix your registered seal or provide your signature in the "Registered Seal or Signature" field at the top of the form.
If you have lost or forgotten your registered seal or signature, please use a new seal or provide your signature.
3. Circle the changes in the middle section of the form
In the middle section of the form, please circle the items that you wish to change.
4. Fill in only the items with changes at the bottom of the form
At the bottom of the form, please fill in only the new information.

Customers who meet the following conditions will need to complete the procedure using a separate dedicated form.

・Customers holding a Monex Securities or SBI Securities brokerage account
・Customers holding Maruyu (special) accounts
・Customers appointing a proxy in Japan due to relocation overseas
